Please include your email and/or phone so that we may contact you in the event of a needed change to the Mass intention.

If we cannot reach you, we will arrange it for the next best date available.

Please include the name and address for any Mass Cards you would like sent. 

Please indicate how you would like your name to appear in the Mass card  if different from above. Example:  "The Smith Family", "Mr. & Mr. Smith", etc.
